Our list of participating insurances may change from time to time, so we will review your insurance when you come for your first appointment. If you have a question about whether or not we participate with your insurance, contact your insurance prior to your appointment.
If your insurance changes, please be sure to notify us as soon as possible. Having accurate information enables us to submit claims accurately. You should also review your insurance yearly to see if there are any changes that you need to be aware of.
If your insurance requires a referral for your visit, it is your responsibility to obtain the referral from your primary care physician prior to your appointment.
If your insurance has a co-pay, your insurance requires that we collect your designated co-pay at the time of service. Please be prepared to pay the co-pay at each visit.
Please give us 24 hours (one business day) notice if you are unable to keep your appointment.
We understand that sometimes the bill cannot be paid in full at the time of service. It may be beneficial for you to set up a monthly payment plan to keep you current with our office. Please contact our billing department to discuss payment arrangement options.
Bayside Orthopedics reserves the right to utilize a third party collection agency for account balances not settled in a timely manner. Failure to keep your account current may result in dismissal from the practice. This may also result in owing collection costs, attorney fees and court costs for delinquent accounts.
